Synthtopia has a detailed write-up on the new Lunar Module optical MIDI theremin from ElectroKraft. The unit can be "customized before or after shipping" to produce various MIDI control signals. Not sure if that means re-programming, flipping a switch, or plugging in a chip, but hey... it's flexible.
